Please note that
(a) Your name, organisation, and email address will be included in a participants’ list for distribution at the event;
(b) Photographs and recordings made of your voice or image during the event might be used, reproduced and distributed for educational use purposes;
(c) AMSI will keep you informed about upcoming events by emailing you the AMSI bulletin (unsubscribe option available).

If you do not agree with (a), (b) or (c), please indicate your disagreement in "Remarks".

AMSI and the University of Adelaide respect the privacy of individuals and acknowledge that the information that you provide on the form is personal information as defined by privacy legislation. The intended recipients of the information are the Event Organisers and any service providers as required (for example, accommodation providers).
